In the originally published version of this manuscript, there was an error in the final sentence of the results section.
The passage should read: “…and was lower in smokers compared with non-smokers [OR = 0.71 (95% CI: 0.57, 0.89)]” instead of “…and was lower in smokers compared with non-smokers [OR = 0.71 (95% CI: 0.57, 0.49)].”
This error has been corrected online and in print.
