The urgency of the situation favors a repurposing of active drugs but not only antivirals. COVID-19: A Centennial Pandemic from Origin to Clinical TrialsIn December 2019, an unexpected interaction of coronavirus with human’s occurred for the third time in history after Severe Acute Respiratory Syndrome (SARS) in 2002-2003 and Middle East Respiratory Syndrome (MERS) in 2012. COVID-19 PANDEMIC: CHALLENGES IN ADMINISTERING CLINICAL TRIALS2022 •
Extraordinary challenges are faced globally due to COVID 19 pandemic, as nations tell the tale and overcome economic challenges. The lockdown imposed in several international locations has also affected the overall medical trials due to travel restrictions, online personnel unavailability, investigational product unavailability, and scientific oversight. Multiple complex challenges involving various ambiguities and factors, including prioritising COVID-19 affected patient protection and treatment during the virulent disease, enrolling members into studies, maintaining patient adherence, providing up-to-date health information through diagnostics and administrating interventions and treatment therapies to patients who have already been registered, have highlighted the challenges and raised various scientific and moral issues associated with scientific research. The modern scenario has caused well-timed trouble fixing to warrant and safeguard patient safety in scientific studies and is observed and maintained.[ 1] The fundamental reason is to spotlight critical problems in administering scientific trials during the pandemic and provide structured tips for promoters, examiners, and members participating or part of the trial to figure out plausible dangers and challenges to efficiently generate high-quality scientific evidence. Corona viruses are a large family of viruses that cause illness ranging from the common cold to more severe diseases. A novel coronavirus (nCoV) is a new strain that has not been previously identified in humans. On 30 January 2020, the WHO Director-General declared the novel corona virus (COVID-19) outbreak a public health emergency of international concern. Many fatalities have occurred and continue to happen. Despite staggering research efforts for therapeutics, complete cure is still a distant dream. The write up updates the treatment approaches utilized in clinical settings across the globe and the clinical trials registered in the Federal register and WHO register for drugs and vaccines to combat the pandemic.

The DisCoVeRy clinical trial aimed at the evaluation of four treatments for patients suffering from severe to critical COVID-19: Hydroxychloroquine, eventually associated with azithromycin; the combination lopinavir/ritonavir; the combination with the addition of interferon β-1a; remdesivir. The trial was discontinued due to the lack of positive results. Meanwhile, many other potential options have been considered either to target the virus itself, the interactions with the host cells, or the cytokine storm frequently observed during the infection. Several of those options are briefly reviewed. They include vaccines, small molecules, antibodies, and stem cells.

COVID-19: An Update about the Discovery Clinical TrialFinding efficacious and safe treatments for COVID-19 emerges as a crucial need in order to control the spread of the pandemic. Whereas plasma therapy attracts much interest, the European project Discovery focuses on the potentialities of small molecules like remdesivir, the combination of lopinavir/ritonavir, hydroxychloroquine, and chloroquine. Results recently published on the clinical evaluation of those drugs are compiled in this brief report, although complete data are still impatiently awaited.

Through this emergency, is it possible to look at the current clinical trial processes with new eyes?
